Sample notes

What the note
actually looks like

Six dental visits, each shown as the conversation excerpt and the note CareNotes wrote from it, in the same note viewer the app uses. Fictional, de-identified, and unedited, so you can judge the output instead of the marketing.

How to read these

The transcript is an excerpt of the on-device speech-to-text; it is the only thing that left the phone. The note is the first draft, before the clinician edits and signs, shown in the live note viewer from the CareNotes web app: click a section to edit it, tap a pill to change a choice. The Progress Note template (Pt CC, RMH, consent, Exam, Tx, NV, plus a Side Note for personal details) is the default; your office's own templates replace it. Nothing in a note is invented from outside the recording, so a missing dose or step in the transcript stays missing in the note until the clinician adds it.

For how the pieces fit together, see the AI dental scribe page and the technical specification.
